• Telehealth involves the use of electronic communication to provide care remotely

• There are inherent limitations, including the inability to perform a full physical examination

• Technical issues may occur that impact communication

 

You agree to participate in telehealth services and understand that in-person care may be recommended when clinically appropriate.
